As the parent of a student enrolled at Academy STAR Dance Studio ("the studio") or the student enrolled at the studio, I recognize that dancing, aerobics, acrobatics, gymnastics and any other activities taught by the school contain various risks to physical well-being.

However, in consideration of the instruction made available to the student listed above at the rates set by "the studio”, I hereby release “the studio” from liability, and knowingly waive my
right to seek damages from the school, for any and all instances of physical harm to that student’s well-being resulting from the ordinary activities of the studio, unless the activities are conducted without regard to the safety of that student.

Permission is also granted to the school to publish photographs taken of students while in association with the studio. These photographs may be used for publicity purposes indefinitely and it is understood that no modeling fee is due to the student.
